股骨粗隆间骨折人工全髋关节置换术

【手术适应证】

1．一般应在80岁以上。

2．股骨粗隆间粉碎性骨折（EvansⅢ～Ⅳ型）。

3．股骨近端严重骨质疏松不适合其他内固定者。

4．骨折前就有股骨头坏死或骨关节炎而有全髋关节置换适应证者。

5．不能耐受二次手术的高龄患者。

6．伴有神经、运动系统疾病：如偏瘫、帕金森病、老年性痴呆等；伴有精神或智障患者。

7．并存病经过对症处理相对稳定者。

【手术禁忌证】

同股骨粗隆间骨折人工股骨头置换术

【术前准备】

同股骨粗隆间骨折人工股骨头置换术。

【麻醉方式】

同股骨粗隆间骨折人工股骨头置换术。

【手术操作】

手术切口、显露等基本操作与股骨粗隆间骨折人工股骨头置换术相同，所不同的是增加了人工髋臼置换。骨水泥型全髋关节置者一般在术后1周左右也可扶助行器在有人保护下离床活动（图9-28，图9-29）。

【术后处理】 同股骨粗隆间骨折人工股骨头置换术。
